Chronic Care Model (CCM), based on IoT to improve the management of health institutions and the care of patients with type 2 diabetes

Abstract


Chronic degenerative diseases pose a serious risk to health. Diseases such as diabetes are increasingly having a larger number of patients. The integration of technological elements allowed patients to improve their quality of life and health institutions to increase the efficiency and effectiveness of their resources. For this article, we have done a systematic review of the literature to observe and analyze the main contributions of the Internet of Things and Social Technologies in health institutions (to improve the management of their resources, under three approaches: costs, quality and Sustainability); And with regard to the care of patients with diabetes mellitus 2 (under three approaches: prevention, diagnosis and treatment). In last section, due to the integration between the chronic care model and the study carried out we propose some strategies that could allow health organizations to improve their management and improve the quality of life of patient, with some expected benefits.
